Club registration fees are $10.00 per player for the fall and again for the spring. State fees are payable to FYSA at the time of registration and roster approval. Club fees are payable to GSSC, Inc. and are sent to the Director of Competition.
Reasons and Costs for Completing a Change of Status (COS) Form
ERROR: Correct any administrative mistakes of player/coach's pass information (spelling, address, etc.) Must be accomplished within 30 days of receipt of permanent cards.
TRANSFER: Transfer a player from one team to another within the club, or to a different club team, or to a different district, etc. Transfer is a movement.
DUAL REGISTER: Register a player to another team without releasing from club team.
VERIFICATION: Change an unverified player to verified (must have an acceptable proof of birth date attached.)
REPLACEMENT OF LOST PASS: Replace a single player's pass or entire team's passes.
OTHER CHANGE: Such as change of address, phone number, etc.
RELEASE: Drop a player from a team/roster.
All COS forms, except lost pass and change of address, require the old pass be submitted with the form!
